We offer personalised support for your loved ones

Should your family member/whanau wish to be cared for in their own home for their last days of life, we can provide support through the provision of working with our community support workers and the wider the multidisciplinary teams such as Hospice to deliver palliative care support where and when as needed. We prioritise your call and will respond quickly.

Once we receive your call we will advise our Registered Nurse who can contact you to make a suitable time to arrange a visit and provide an assessment to implement a Care Plan, this personalised support plan is tailored to meet your loved one’s individual needs, and is regularly reviewed with the client family/whanau and/or team to ensure that our support service reflects flexible palliative care service in accordance with the way that fits into your loved ones life style.

We listen, value family/whanau input and work together to implement our client and family/whanau individual needs. We acknowledge the differences in individual cultural perspectives and care for our clients, respectfully. Our team provides care and companionship to clients and family/whanau providing ease of access for families to have time out when they need to.
